- Greetings - And Welcome to Death Warrant ! -
Firstly, you must make sure your mic is working using only windows explorer, test it for sound coming out of your speakers so that you can hear yourself , if passed this step , then you work it on UT.
By default in UT you should be able to talk and hear others, theres also a keybind by default to activate your mic for voicechat , wich is the letter 'F' . Now if you changed these default settings, Just make sure you have allowed Voice chat, and set it's volume to a proper level , then Bind a Key to activate your mic, choose whatever key would be more confortable to be used for this, can be also a mouse button if you have plenty , to bind it, you run UT, open the Settings , then select Input , then clik on the 'Configure Controls' Button , scroll down, to the 'Communications' section, and make a left click to the right of the ( Under the K1 tab) 'Activate Microphone' line , then push the key or button that you have decided to use to activate your mic in game, and your selection should appear there, ; you're all set, Now just make sure you clik on the 'Close' button at the bottom of that Settings page . Whenever you are in a game, just press and hold that button while you talk , it becomes a ppt ( push to talk) key!
Note: If you already know and tried this , and it doesn't works , then start by letting us know what OS you have..
HTH ..

Only 3 people out of 130 veiws that can help me...
As good as the UT2004 voice-chat seems, Ventrilo will blow it and almost any other voice-chat program away. If you really want to communicate properly with teammates and such, you should download Ventrilo (make sure to download the client version, and choose whichever operating system you're using)
and then sign into the DW server using the following information:
Hostname or IP: sprint.typefrag.com
Port number: 19274
Password: clandwpublic
Ventrilo gives you lots of options in terms of how you can talk to others and what you hear, has several different channels to choose from, and is generally less of a headache than the UT2004 voice chat.
Oh, and make sure to use your UT2004 player name, so we don't get you confused with someone else.

Before troubleshooting in UT, i would see if you can even record in windows. Try going to Start\Programs\Accessories\Entertainment then the Sound Recorder. Start recording and see if it picks up anything you are saying. If nothing is picked up then go to Control Panel, Sounds and Audio Devices. Go to the Audio tab and check the Sound Recording area, and make sure that your device is selected. If it says none, you will need to download drivers for your sound card. If it has something selected like Sound Max, or SB Audigy 2 ZS... then hit that volume button. Make sure your volume is turned up on the microphone and also exam that check box below it. If it says [] Select, make sure it is checked, if it says [] Mute, make sure it is NOT selected. You may also want to check that Voice tab and see what it says for recording device, let us know what your results are after that.

Hey ya CyberClown!
Unfortunately, the in game chat in UT is a fickle beast for some of us. For me, about half the time, I can't hear people who're talking and a lot of the time they can't hear me either. If you've followed all of aLpHa's and Namu's suggestions, and it still doesn't work, perhaps you could go to your UT settings info/Audio and post here exactly what boxes you have ticked so we can compare notes.
A note on Ventrilo: Not all players use this fine VOIP program. There are a lot of people in Monster Mash who do, though and I'd recommend it to use in addition to the in-game UT chat. There is a step by step tutorial on how to set it up here: Getting Started with Ventrilo
Don't get discouraged! We'll do what we can to get you "on the air"!
